• A Crane Lift Notification Form (with attachments) must be submitted prior to any crane mobilization within Mashantucket.
• Projects involving a Critical Lift (see page 1) require Approval and must be submitted at least 5 days prior to crane mobilization.
• If a Critical Lift is involved, obtain Approval via returned emails or hardcopy initialed form.
